If your pet arrives at Dopamine K9 with ticks or fleas, owner agrees for treatment at an additional charge. Capstar orally and a flea/tick bath are the products we administer.

I understand and accept that Dopamine K9 takes all precautions to keep pets safe and healthy. They shall not be held liable for or in the event of loss or damage due to an "act of God", natural disasters, or circumstance beyond their control including, but not limited to, disease, death from natural causes, running away, theft, fire, self inflicted injuries, or injuries by other animals.

Pet Owner agrees to pay per calendar day during pet's stay at Dopamine K9. Payment is due in full on the date the dog is scheduled to return home. Owner will be charged for the days reserved and any additional days added.

Owner agrees and understands that in the case of an emergency, circumstances may not permit enough time to contact listed contacts before proper care needs to be provided. Owner grants to Dopamine K9 complete authority to make decisions involving medical treatment of said pet, including but not limited to, obtaining medical treatment from any certified Veterinarian. In the event that a Veterinarian deems it an unrecoverable condition and the pet is suffering and all contacts have been tried but proven unsuccessful, owner gives permission for Euthanasia.

Owner understands and agrees that if their dog is not picked up within 7 calendar days after the scheduled pick up date, the dog shall be deemed to be abandoned and additional boarding charges shall apply, and in such event, Dopamine K9 shall gain legal custody and ownership of the dog and retain the right to keep the dog or place the dog in a shelter or with animal control, or place the dog in a new home with a new owner, with no recourse by owner against Dopamine K9 or any Dopamine K9 party. Owner FULLY UNDERSTANDS AND AGREES THAT IF OWNER ABANDONES HIS/HER DOG AT DOPAMINE K9, OWNER IS FULLY RESPONSIBLE FOR ALL BOARDING FEES FOR AS LONG AS THE DOG WAS IN THE CARE OF DOPAMINE K9.
